Our Pediatric Caregiver survey is intended to collect community information from caregivers of kids under age 18, who have Long Covid, in the United States.
The survey will take approximately 10-15 minutes to complete and your responses will remain anonymous.
Please only complete this survey once, unless you have more than one child with Long Covid.
The collective findings will be shared via the Long Covid Families website and social media channels. This information may be shared in creative formats like graphs or memes, to raise awareness around identifying and understanding Long Covid in children. Participation in the survey is voluntary, and by doing so you agree to sharing unidentifiable information to raise awareness for Pediatric Long Covid.

We are Long Covid Families, a 501c3 nonprofit organization in the United States. We help and caregivers, children, and individuals who have been impacted by long-term complications from COVID-19 infection. We offer support, education, and work as advocates to create change.
